PURPOSE: Council approval and authorize the Executive to sign the Washington State Department of Social and Health Services AAA Hospital Surge Care Transitions Grant
BACKGROUND: The AAA Hospital Surge Care Transitions Grant provides for a contractor to utilize existing frameworks to develop new partnerships and referral protocols with local hospitals directly or coordinating as needed with the local Home & Community Services (HCS) regional office referral protocols. Contractor will accept referrals for acute care discharge and assist with and support care transitions from hospital to home. This may also include referrals from HCS for clients that did not qualify for Medicaid LTSS and require care transitions provided by the Contractor.
This grant is for the period 2/7/2022 to 9/30/2024 in the amount of $85,286. This grant was not included in the Human Services Grant Work Plan, thus requires Council approval.
