PURPOSE: Enter into agreement with Ideal Option PLLC to provide peer navigation services. This Agreement is the product of County RFP No. 24- 007JM Substance Use Peer Navigation Program.
BACKGROUND: This project will provide care and support to individuals in Snohomish County using substances who might also be incarcerated, homeless, or have recently experienced an overdose (the “Priority Population”) via two (2.0) full-time-employment (FTE) Peer Navigators. Navigators will collaborate with clients to achieve clients’ personal goals, which may include reducing harm while using substances or establishing recovery from substance use. Additional outcomes include documentation and reports on client engagement and linkages to/retention in care.
