PURPOSE: Council approval and authorize the Executive to sign the Washington State Department of Social and Health Services Money Follows the Person (MFP) Grant
BACKGROUND: The Money Follows the Person (MFP) Grant provides funding for the County to partner with local hospitals to accept acute care discharge and long-term care referrals and assist with care transitions from hospital to home or rehab facility to home within Snohomish County. This may include referrals from HCS, TXIX Case Management Program, acute care facilities and long-term care facilities for individuals with or qualifying for Medicaid benefits.
This grant is for the period 7/1/2021 to 12/31/2023 in the amount of $195,000. This grant was not included in the Human Services Grant Work Plan, thus requires Council approval.
