PURPOSE: Sign Amendment No. 1 with Swedish Health Services to perform follow-up with overdose patients presenting at Swedish Health Services - Edmonds and Mill Creek locations.
BACKGROUND: The Health Department were awarded the Overdose Data to Action: Limiting Overdose through Collaborative Actions in Localities (OD2A: LOCAL) grant through CDC. The budget period is 9/1/23-8/31/25. Swedish Health Services are named in the CDC Grant Notice of Award to perform these services. Swedish collaborated with Snohomish Health District and the Health Department would like to continue this partnership.
