PURPOSE: The purpose and intent of this Agreement is for Connie Haley, MD, MPH, to provide coordination of care for patients with active and latent tuberculosis disease.
BACKGROUND: Connie Haley, MD, MPH, is a highly qualified Tuberculosis (TB) Medical Officer who will assist the Health Officer with coordination of care for patients with active and latent tuberculosis disease as well as evaluating individuals for potential active or latent tuberculosis infection, with a special emphasis on immigrant and refugee populations. She will work with the TB Program Manager and TB Control Program to review clinical information, conduct diagnoses, prescribe treatment plans, monitor progress, and collaborate with other healthcare professionals to ensure the best possible outcomes for patients.
