PURPOSE: Enter into Amendment No. 2 with Ideal Option PLLC to provide peer navigation services. The Original Agreement is the product of County RFP No. 24- 007JM Substance Use Peer Navigation Program.
BACKGROUND: Snohomish County, acting though its Health Department, previously entered into an Agreement with Ideal Option PLLC for the substance use peer navigation program and wish to continue these services. This project will provide care and support to individuals in Snohomish County using substances who might also be incarcerated, homeless, or have recently experienced an overdose (the "Priority Population") via full-time-employment Peer Navigators. Navigators will collaborate with clients to achieve clients' personal goals, which may include reducing harm while using substances or establishing recovery from substance use. Additional outcomes include documentation and reports on client engagement and linkages to/retention in care.
