PURPOSE: The Health Department is re-enrolling in the childhood vaccine program through the WA State Department of Health. As a part of this process the Health Department needs to sign the provider agreement that outlines all the requirements of the program. The Health Department is agreeing to comply with all Centers for Disease Control and Prevention regulations around vaccine storage, handling, and provision. This agreement also covers data sharing between the Health Department and the WA State Immunization Registry to ensure that any vaccines provided by Health Department staff are accounted for and vaccine records are updated.
BACKGROUND: The WA State Department of Health requires any healthcare provider who wishes to obtain vaccine through the Childhood Vaccine Program to complete an enrollment agreement. This agreement ensures that all parties understand the requirements of the program, the requirements to store and handle vaccine appropriately, and understands the ongoing reporting requirements. DOH requires an enrollment packet to be completed annually.
