PURPOSE: At the request of the CDC the Health Department needs to provide an updated budget for our OD2A grant. The grantor requires the designated signatory to sign cover letter.
BACKGROUND: The Health Department is on year 3 of 5-year grant through the CDC. Based on years 1 & 2 of the grant the Health Department is requesting to adjust the budget to better meet the needs of the grant. The award amount of the grant is not changing. The following changes are being requested: Travel: Added calculation details for airfare, per diem, and lodging for each requested trip, and identified staff and positions expected to travel. Other: Moved administrative costs to the "Other" category; these costs are now allocated through the Health Department's direct allocation process as documented in the memorandum uploaded to Grant Solutions. Contractual: Sound Pathways Harm Reduction Center Period of performance has been updated to 9/1/2023 - 8/31/2026 Itemized wellness supplies; contractor will distribute approximately 267 wellness kits. TBD Peer Services (ODAT) Removed parking costs. Ideal Option: Peer Services Added additional justification for salary and fringe. Itemized supplies. Moved cell phones to the Equipment category (two peers will be issued phones). Itemized wellness kit contents; contractor will distribute approximately 160 kits. Removed COP facilitation and rent costs; will instead provide gift cards to speakers. Removed parking costs. Removed Courage to Change as a subcontractor. Ideal Option: Mass Spec Itemized supplies
