PURPOSE: PURCHASE MGR SIGNATURE APPROVAL ON THIS CREDIT MEMO FOR JOC CONTRACT JC-23-18-F MCPHERSON MITIGATION SITE
BACKGROUND: Original JOC contract with Forma, for the McPherson Mitigation Site work, was for $108,015.47, plus sales tax. PO 36066 was set up for this amount back on 8.25.2023. On 7.9.25 we received a COP#01 from Forma for a credit for work not performed, in the amount of -$59,141.08, plus sales tax. Therefore we are requesting to have PO 36066 reduced by -$59,141.08, plus sales, for a total reduced contract amount of -$64,996.05.
